We recognize the insight you have into your client and value the contribution that providers like you bring to the intake, treatment, and discharge process.Before admission, our assessment team will work with you to collect information about your client to help our senior clinical team make an informed treatment recommendation. We believe that communication is key, and we will establish preferred methods of communication to keep you engaged throughout the admissions and treatment process.

Working Together for Better Outcomes

Once your client has been admitted to NorthStar Transitions, their primary clinician will contact you within the first 72 hours to provide updates and discuss your involvement in the treatment process. We invite referring professionals to participate in staff meetings with our team to provide an opportunity for you to share clinical insights and stay informed about your client's recovery progress. We believe that referring provider involvement allows for a seamless transition to outpatient care following discharge from treatment, and we are committed to working with you to ensure the best possible outcomes for your clients.

Aftercare Planning

NorthStar Transitions provides detailed aftercare planning to ensure the success of each client following treatment. Our staff works together with referring professionals to recommend and coordinate the right level of support a client needs to continue their recovery journey. This may include outpatient treatment, sober living, ongoing drug and alcohol testing or alumni programming. We also provide a comprehensive review of the treatment the client has received and the discharge plan to ensure continuity of care.

Admissions Process for Referring Professionals

NorthStar Transitions can provide a higher level of care for your clients experiencing drug or alcohol addiction. As a referring professional, your involvement begins during the admissions process and continues throughout treatment to ensure that your client’s needs are met. This also helps us better understand their substance abuse history, mental health needs and more and aids in the development of a truly individualized treatment plan.

  • We verify insurance benefits and discuss coverage/cost of treatment
  • Pre-admission assessment is performed with the prospective client
  • The results are reviewed by our assessment team and the referring provider to determine if our treatment programs are an appropriate fit
  • If the client is accepted, a service agreement is signed
  • Admission date and time are scheduled at least 24 hours in advance

Yes, we accept most major insurance plans. Our team can work with you and your client to determine insurance coverage and benefits.

How can I stay involved in my client's treatment?

NorthStar Transitions is committed to collaborating with referring professionals to ensure their clients receive the best quality of care possible. Many providers request periodic status updates or progress reports by phone or email, while others join our staff meetings throughout the course of treatment. When it’s time for discharge, our aftercare coordinator will contact referring professionals to discuss the recommended amount of ongoing support and help ensure continuity of care.
